Most people do not!
I see this all the time in company Quality Management Systems (QMS) - information is in the wrong document.
This leads to confusion and a lot of duplication!
Think about the types of information - who, what, when, where, why, how. Understanding this can be SO helpful in knowing where information is best placed in your QMS.
Procedures - describe the end to end process, may be done over time by more than one team or group, and should contain who, what, when, where details.
Policies - should be short and not include any procedural information, mostly why, when and maybe who. If your policies contain process details, you are duplicating effort and information.
Instructions - should be detailed how information completed by one person or team in one sitting, and should have a parent procedure.
